North Perth Community Hospice is here to provide compassionate support.

A Little History

North Perth Community Hospice was incorporated in 1993. It is governed by a Board of Directors and partially funded through a service agreement with South West LHIN and the Government of Ontario. Forty percent of the operating funds are raised through community donations and fundraising events and campaigns.

Our Mission Statement

The goal of hospice care is to uphold the quality of life.

The hospice care team of professionals and volunteers recognizes the individual as a whole person and strives to meet his/her physical, psycho-social and spiritual needs.

Hospice care addresses the needs of both the individual and the family.

The individual has the right to make informed choices and decisions based on truth and understanding.

Initial contact may be made by any individual, family, or health care professional. Care will be available throughout the illness and/or grieving process as appropriate.

North Perth Community Hospice will strive to ensure compatibility between the skills, experience and personality of the volunteer and the needs of the individual and/or family.

The hospice program includes ongoing community awareness and education.

Meet our Staff

Susan Qureshi
Coordinator of Hospice Services

Susan provides Support and Counselling services for individuals who have experienced a loss. She collaborates with the Coordinator of Visiting Volunteers to ensure quality care throughout all of North Perth Community Hospice programs.

Kourtney Richardson
Coordinator of Visiting Volunteers-Certified Grief Recovery Specialist

Kourtney coordinates the services of North Perth Community Hospice involved in matching skilled Volunteer Visitors with clients who are facing life limiting diagnosis.

Catherine Corcoran
Office Manager

Catherine Corcoran is the Listowel Office Manager.
