The hospice care team of professionals and volunteers recognizes the individual as a whole person and strives to meet physical, psycho-social and spiritual needs.
Hospice care addresses the needs of both the individual and the family.
The individual has the right to make informed choices and decisions based on truth and understanding.
Initial contact may be made by any individual, family, or health care professional. Care will be available throughout the illness and/or grieving process as appropriate.
North Perth Community Hospice will strive to ensure compatibility between the skills, experience and personality of the volunteer and the needs of the individual and/or family.
The hospice program includes ongoing community awareness and education.
